What services do you offer?
Rachel provides functional capacity assessments and reports for the NDIS. She can also complete progress reports or help you at your NDIS planning review. Rachel provides capacity building support and on going therapy. She can work alongside you, your loved ones and other health professionals. Please refer to our OT services section of this website for further information.
Can you travel to me?
Yes, Rachel completes home visits or sessions in the community within 45 minutes of Graceville Brisbane. Travel is charged at up to $48.50 each way and 0.99 cents per kilometre.
Rachel offers telehealth Australia wide if you are not based in Brisbane. This is usually completed via Microsoft Teams video chat.
Do I need a referral for my appointment?
Yes please. It is helpful for Rachel to have information about your location, NDIS number, plan manager details if applicable, what your goals are and who is in your support team etc. Please click the referral button on this website to fill out your details and then Rachel will get in touch. Your support coordinator, support worker, family member or General Practitioner could help you complete this form if you need.
Can I see you through Medicare subsidised appointments?
Yes Rachel offers medicare subsidised appointments for Occupational Therapy. You can get in touch with your General Practitioner to discuss this and obtain a referral. You will need to complete your medicare claim following the appointment to receive your reimbursement. These appointments can be completed at your home (with travel fee), somewhere in the community that you feel comfortable or in Graceville as Rachel can hire a therapy room as required.
